Eligibility: Sweepstakes only open to legal residents of the 48 states in the Continental United States, 18 years of age or older, except employees and their immediate families (spouses, parents, children and siblings and their respective spouses) of Domino’s Pizza LLC, Domino’s and its parent company, franchisees, subsidiaries, affiliates, distributors, suppliers, advertising/promotion agencies and/or those living in the same household of each. Parents of children with exactly five letters in their first name, middle name and last name may enter on behalf of their child as long as they are over the age of 18. Void where prohibited.

How to Enter: Consumers who have exactly five letters in their first name, middle name and last name (nicknames do not count) should send a copy of their birth certificate, driver’s license or other valid form of government-issued identification along with a mailing address and telephone number to 555names@dominos.com or fax to1-800-615-6119. Parents entering for a child with exactly five letters in their first, middle and last names will be asked to affirm that they are over the age of 18 by submitting a copy of their driver’s license or other valid form of identification along with their child’s identification. Domino's will not retain any copies of information submitted with respect to children, nor will the information be used or published.

Winner Selection: The first 100 qualified entries received before midnight EST, July 23, 2007, will win. Winners will be notified by e-mail and/or mail.

Prize: 100 entrants will receive $20 worth of Domino’s Pizza gift certificates good for use towards any Domino’s menu item. No transfer, assignment, cash redemption, or substitution of prize allowed.

No additional remuneration shall be granted.

Any and all taxes or additional costs incurred as a result of accepting a prize from Domino’s Pizza are the sole responsibility of the winner. Winners assume all liability for any injury or damage cause, or claimed to be caused by participation in this promotion or use or redemption of any prize. By participating in this promotion, entrants agree to be bound by the Official Rules and decisions of the judges. Domino’s is not responsible for any typographical or other error in the printing, the offering or the announcement of any prize or in the administration of the contest.
